This book describes history, pathophysiology of cerebral ischemia, diagnostic procedures, and treatment modalities and possibilities. The treatment possibilities are described in detail, starting with magistral vessels disease, going through to EC-IC bypass procedures, treatment of specific conditions like moya-moya, major randomized trials are the subject of dedicated chapter. Endovascular dealt with in a comprehensive way. This book intends to be the reference source for anybody dealing and/or being interested in the topic. The target audience are neuroscientists neurologists, neurosurgeons, vascular surgeons, radiologists, and anybody dealing with the patients affected by CVD. The readers of recently published books (see below) were always restricted group, the books are targeted on neurologists or neurosurgeons or radiologists. This proposed book is catch all reading. This book  finishes with chapters on future directions and translational research (stem cells transplantation), thus somewhat increasing the life span of the publication.

Professor Vladimír Bene is a neurosurgeon and author specializing in vascular neurosurgery and skull base surgery. He is a former chairman of the Department of Neurosurgery and Neurooncology at Charles University in Prague. He currently holds the position of chairman of neurosurgery at the Postgraduate School of Medicine in Prague. He was trained at home and abroad, notably through a year-long research fellowship under Professor Spetzler at the Barrow Neurological Institute in Phoenix, as well as a period spent with Professor Yasargil in Zurich. He is the past president of the European Association of Neurosurgical Societies, the past president of the European Skull Base Society, and the past president of the International Meningioma Society. He serves on the editorial boards of several journals, including Acta Neurochirurgica and Neurosurgery, among others, and is an active reviewer for many international journals. Professor Bene obtained his Candidatus scientiarum degree for a thesis on experimental spinal cord injury. His assistant professorship thesis was on acute surgery of cerebral aneurysms. His Doctor scientiarum degree was awarded for a thesis on arteriovenous malformations of the brain, and his full professorship was based on a book on cerebral ischemia. He was the principal investigator of 22 projects funded by the Grant Agency of the Czech Ministry of Health. He is the editor of 3 books published by Springer: Brain Arteriovenous Malformations (2017), Cavernomas of the CNS (2020), both edited with O.Bradác, Neuroanatomy Guidance to Successful Neurosurgical Interventions (2024), edited with I.Kanaan.
 
Prof. David Netuka studied medicine at the First Faculty of Medicine, Charles University in Prague (1994 2000). Since 2000, he has worked at the Neurosurgery Clinic of the First Faculty of Medicine and the Central Institute of Neurology and Neurosurgery, becoming head physician in 2009. In 2015, he was appointed deputy head of the Neurosurgery and Neuro-Oncology Clinic, and in 2020, head of the Department of Neurosurgery and Neurooncology at the First Faculty of Medicine and the Central Military Hospital. He completed his PhD in Neuroscience (2000 2005) at Charles University, focusing on internal carotid stenosis. He passed board exams in neurology (2003) and neurosurgery (2007), became associate professor in 2010 (topic: cerebral vessel stenoses), and full professor in 2018 (topic: endoscopic skull base surgery and intraoperative MRI).

Prof. Netuka supervises doctoral students in Neuroscience and serves on the neurosurgery certification committee. He has authored 3 monographs and contributed to 18 book chapters. His 145 publications span three main areas: cerebrovascular diseases and their diagnostics, sellar lesion management (especially pituitary adenomas), and intraoperative MRI in neurosurgery. He has long been active in the European Association of Neurosurgical Societies (EANS), serving on the Young Neurosurgeons Committee, the Individual Membership Committee since 2009, and currently as Board Member for CME and Guidelines. He lectures at EANS courses globally. He is on the editorial board of Neurosurgical Review and reviews for Acta Neurochirurgica, World Neurosurgery, Journal of Neurological Surgery A, and Brain and Spine.
