Adipose tissue (also known as fat tissue) is a human tissue with a high concentration of adult mesenchymal stem cells (ADSCs), the proven regenerative capacities of which have now applications in various medical fields as reconstructive surgery, dermatology, scar therapy, orthopedics, vascular surgery, cardiology, gynecology, otolaryngology, proctology, as well as in antiaging therapies in cosmetic surgery or aesthetic medicine. Regenerative therapy has proven very promising in face rejuvenation, where skin atrophy and volume loss, the main factors beyond facial aging, cause facial rhytids, laxity, skeletonization, and pseudo-descent of the midface.
This handy guide offers a new standardized, guided procedure, the Superficial Enhanced Fluid Fat Injection (or S.E.F.F.I.), that will enable professionals dealing in an outpatient setting to perform safe, effective tissue rejuvenation, increasing the patient s compliance, guaranteeing effectiveness, reducing risks and costs. Harvesting subcutaneous adipose tissue is a critical process without a specific training, exposing the patient to possible complications related to incorrect harvesting maneuvers. The guided SEFFI procedure allows to perform harvesting at a safe plane and 15mm depth, explains how to restore volume and regenerate the skin injecting viable adipocytes, by adhering to a rigorous clinical protocol, following steps in images and videos.
In particular, chapters deal with making tissue harvesting less traumatic, collecting cells in the most superficial plane to gain a richer tissue in terms of Stromal Vascular Fraction Cells (SVFCs) and mesenchymal stem cells (ADSCs), and harvesting the cell clusters containing adipocytes, SVFCs and ADSCs of reduced size, avoiding fragmentation devices and unnecessary manipulation.
Associated treatments (Calcium Hydroxyapatite, Energy Based Device, Bio-Photostimulation) for combination with the SEFFI procedure, and it application in reconstructive hair transplantation and remodeling after injury. Domenico Piccolo, MD, is Dermatologist, Head of Dermoaesthetic Laser Centers Novea in Avezzano, Pescara and L'Aquila, Italy. His expertise includes videodermoscopy, skin melanoma prevention, scar treatment, and lasers applied to skin and aesthetic treatments, for which he owns patent. After specializing in Dermatology in 2000, he focused on pigmented skin lesions, photodynamic therapy, skin surgery and laser applications, both in dermatology and aesthetic medicine. Resident with Prof Peter H Soyer (1997-99 Graz, Austria), in 2009 he registered as UK practitioner (General Medical Council - GMC), at the Skinfinity Cosmetic Clinic, Leeds. Currently adjunct Professor, Dermatology Clinic, University of Trieste, Florence, and Pavia. Lecturer at the Master on Lasers, Sapienza University in Rome. Dr Piccolo is the President of the Italian Association of Ambulatory Dermatologists (AIDA) and Past vice-president of the Italian Society ofLaser Dermatology (SILD). Author of 180 papers, 5 books and many book chapters, speaker at 500 National or International congresses.
Alessandro Gennai, MD, is a Reconstructive and Aesthetic Plastic Surgeon based in Bologna and Milan, Italy. He graduated with honors in Medicine and Surgery in 1988 and completed his specialization in General Surgery at the University of Modena in 1993. In 1994, he concluded the International School of Aesthetic Medicine (Fatebenefratelli Foundation, Rome), followed by a private fellowship in 1999 at the ISSE Institute in Los Angeles under Prof. Nicanor Isse, a pioneer of endoscopic facial techniques. Dr Gennai is internationally recognized for having developed and standardized three original surgical and regenerative techniques: MIVEL (Minimal Incisions Vertical Endoscopic Lifting), SEFFI (Superficial Enhanced Fluid Fat Injection), and NAL (Neck Artificial Ligament), all published in leading journals including OPRS, ASJ, JAMA Facial Plastic Surgery, and the European Journal of Plastic Surgery. His work focuses on minimally invasive facial rejuvenation and regenerative medicine with adipose-derived stem cells. He has authored and co-authored over 40 scientific papers and 12 books, and frequently lectures at major national and international congresses. Dr Gennai is a member of AICPE, SICPRE, SIME, and serves as contract professor in several second-level university master s programs in Italy. Since 2017, he has been on the Educational Board of the European Academy of Facial Plastic Surgery (EAFPS).
Dr. Fabrizio Melfa graduated in Medicine and Surgery from the University of Palermo in 1994. He specialized in Nutritional Sciences at the same university in 2000 and, two years later, in Aesthetic Medicine at the SIME-FIF in Rome (with Professor C.A. Bartoletti) and, subsequently, at the University of Pavia. In 2005, he earned a Master's degree in Phlebolymphology from the University of Siena. In 2013, he completed an advanced course in Skin Health Restoration (with Dr. Zein E. Obagi, Beverly Hills, CA, USA). Dr. Melfa is currently the founder and director of the MediAging medical-scientific program and an adjunct professor at the University of Pavia and the University of Genoa, teaching the Master's program in Aesthetic Medicine. Dr. Melfa has co-authored national and international publications and has organized events dedicated to phlebology and Aesthetic Medicine and Surgery.
Bruno Bovani, Md, specializes in General Surgery and acquires a Research Doctorate in Experimental Surgery at the University of Perugia. He then worked as a plastic surgeon at the Department ofPlastic and Reconstructive Surgery, University of Perugia until 2002, after which he continued his activity in Outpatient clinics.
