Preface


It has been seven years since the Third edition of Bostwick’s Plastic and Reconstructive Breast Surgery was last published. In the interim, techniques have changed and advances have been made which continue to impact the practice of breast surgery on a daily basis.

In addition, my own experience has expanded and I have been privileged to be involved in the forefront of developing some of the new techniques in pre-pectoral breast reconstruction and perfusion imaging. Implant technology is evolving and advances in microsurgery and lymphatic surgery continue to impact our patients. This wide array of factors prompted us to consider revision of this classic text, if only to make it a better resource for fellow surgeons and the patients we treat.

John Bostwick was a man who impacted my life greatly, and it is an honor and a privilege to remember his name in association with the book that he pioneered so many years ago. I met him for the first time in 1988 when I joined the fellowship program in the section of Plastic Surgery at Emory University. The year I spent working with John, Rod Hester, Foad Nahai and Josh Jurkiewicz transformed my life and career as a surgeon. They ignited my passion for breast surgery and changed the way I think about surgery and problem solving. Dr. Jurkiewicz was an inspiration as he sought to encourage his faculty to “think outside the box” and follow their creative instincts. John was a prime example as he sought answers to the questions posed by clinical practice. His original first edition of this book was the combination of that thought process and the realization that there was a need for a comprehensive work on plastic surgery of the breast that addressed not only the academic issues surrounding the management of breast disease but also the technical processes and nuances involved in achieving a successful outcome.

After returning to a private practice in Cape Town in 1990, I was invited back to join the faculty at Emory in 1992. John assumed the position of Chairman after Dr. J’s retirement. When planning the Second edition he asked me to write the section on microsurgical breast reconstruction as it had become a significant aspect of my practice at that time. Through the Second and Third editions of this book, Bostwick’s Breast Surgery continues to be a leading text in plastic surgery and is regarded by many as the “Bible of breast surgery” as it has been called numerous reviews over the years, becoming a key component of many plastic surgeons’ libraries. Following John’s untimely death in 2001, the Second edition continued to be a best seller but revision was clearly necessary. The Third edition followed and has continued on the success of the earlier volumes. Those publications would not have been possible without the dedication and guidance of Karen Berger and her team at Quality Medical Publishing in St Louis Missouri.

Karen asked me to rewrite and edit the Third edition of the book. It was a daunting task approaching a classic text such as this while being true to the memory and mission of its original author. John Bostwick was an extremely talented and experienced breast surgeon whose insights into our discipline raised its profile in women’s health and advanced the frontiers of plastic surgery as a specialty. Many of his observations and approaches to breast disease remain as relevant today as they were at the time of publication of both the earlier editions. The Third edition built on that knowledge base and expanded the topics covered.

When Karen Berger retired, the rights to the book were ultimately acquired by Thieme Publishers based in New York and Germany. Sue Hodgson became the new publisher and she approached me to consider writing a Fourth edition to update and expand the book as it approached its seven-year renewal cycle. Having only just finished writing the two volume set “Reconstructive Surgery: Anatomy, Techniques and Clinical Applications” a few years before, I was somewhat reluctant to embark on yet another massive textbook revision! My practice was busy as was life in general. Fortuitously, I had become involved in advancing the concepts of prepectoral breast reconstruction as well as being involved in new imaging technology for perfusion assessment, and with the additional advances in microsurgery, lymphatic surgery and changing views of implant technology, I felt that a major revision was unquestionably called for.

One of the criticisms of all of the previous editions, was the large chapter size and difficulty navigating the text in order to arrive at the section in question. While the information was there it was often difficult to find at a glance. Some of the images remained from the original first edition and although many of these were pertinent and informative, a large number of them needed updating. The microsurgical section, while covering the important flaps used at the time, was now deficient since the introduction of more perforator flaps. The entire section was in great need of incorporating more technical detail so that the reader could come away with a clear understanding of the intricacies involved in successfully performing some of the more difficult perforator procedures. Expanded video coverage of these procedures was required and the importance of preoperative imaging needed further elucidation.

In addition, new horizons had opened up in micro-lymphatic surgery, which is beginning to exert a profound effect on both lifestyle and clinical outcomes for patients suffering from post-mastectomy and post-radiation lymphedema. For the first time, the management of lymphedema is being measurably impacted by advances in micro lymphatico-venous surgery and lymph node transfer.

One of the most significant upheavals in implant-based surgery has been the progressive realization that the pre-pectoral approach offers significant advantages over the traditional sub-pectoral approach and may indeed become the implant position of choice in the future for breast reconstruction. It has also become apparent, that with appropriate perfusion assessment, single stage direct-to-implant reconstruction can be performed without the use of two-stage expansion. The approach is relatively simple and there is none of the patient discomfort, cost and morbidity of two-stage expander-implant reconstruction. No textbook in publication at this time covers these important changes.

While it is pertinent to observe the changing face of breast surgery, it is even more salutary to note the manner in which information dissemination is evolving. The third edition text was one of the first large volume textbooks to incorporate multiple DVDs containing video footage of the procedures embodied within the text. Since that time, digital learning has exploded and DVD technology is progressively being replaced with online video access formats. As we planned this Fourth edition, it became apparent that the younger readership we are targeting is more used to online access as well as more focused visual learning. Having critically reviewed the prior edition, we felt that the approach to this Fourth edition would require not only an expansion of the information covered but also a complete change in the format of the book. With those goals in mind we have targeted online video and access to the text itself, which will allow surgeons to easily access information in hospital or clinic settings using computers, cell phones and tablets without having to have the book physically in their hands.

One of the major changes to the book has been a change in its basic format. While there are still large blocks of text, which are unavoidable when expanding upon the nuances of a given procedure, the text has been broken up into many smaller chapters covering individual topics. This is a major digression from the original format, which grouped multiple procedures under one heading such as mastopexy or breast reduction for example. Hopefully this will enable the reader to navigate the text more readily.

With regard to content, we have retained the essential format of Volume One being primarily aesthetic in focus with Volume Two focusing on reconstructive surgery. Volume One has more in-depth operative detail on all aspects of esthetic breast surgery and includes a comprehensive overview of breast implant-associated anaplastic large cell lymphoma. The sections on fat grafting have been expanded to be more data driven. Chapters on preventing and managing complications of the various esthetic procedures performed are included. In Volume Two, the surgical oncology chapters have been updated and include more detailed sections on imaging and pathology as they relate to breast cancer diagnosis. Perfusion assessment techniques are discussed in some detail as new imaging modalities are appearing on the horizon. The basic science and role of acellular dermal matrices in the prevention of capsular contracture and implant pocket control, particularly in pre-pectoral breast reconstruction, are discussed at length. Given the explosive interest in pre-pectoral breast reconstruction and pre-pectoral conversion, several chapters are devoted to exploring this fascinating topic. The oncoplastic surgical section has been expanded and updated to incorporate the latest techniques used by both plastic surgeons and breast surgeons to minimize breast deformity and radiation complications. The book truly represents the global state of the art, with contributors from over 7 countries.

In preparing this fourth edition, I have sought to continue to honor the memory and pioneering spirit of John Bostwick whilst forging a completely new direction with the text. It has been a daunting task. My ultimate goal in writing...
