This program demonstrates a surgical technique associated with acetabular defect reconstruction with modular metallic augments. Indications for this technique include the treatment of non-contained acetabular deficiencies, cautious use is suggested with pelvic discontinuity, and contraindications include active infection. The video highlights acetabular bone preparation, augment sizing, bone grafting, augment fixation, and hemispherical cup placement. The surgical technique begins with acetabular exposure and protection of the neurovascular structures. Fibrous tissue is removed from the acetabular bed, exposing bleeding bone. The remaining superior, inferior, and anterior bone is assessed. The teardrop is identified and serves as an inferior cup-positioning landmark. Gentle reaming in the anatomic hip position re-establishes the center of rotation. A trial acetabular shell is placed for sizing and positioning. Trial augments are tested to create a stable construct in the superior-inferior and anterior-posterior planes. Augments can be placed in various orientations, depending on defects and remaining bone. The augment is fixed with multiple 6.5 mm screws directed in a perpendicular fashion.
Particulate cancellous grafting is placed in contained defects, and a thin layer of cement is placed between the augment and the final acetabular shell. For final stability, additional screws are fixed in the shell adjacent to the augment. A cemented liner with the largest femoral head possible is used to reduce dislocation risk. The video includes a discussion of 43 consecutive cases with no acetabular loosening at their 24-month followup.
