A convincing body of data links depression, cardiovascular disease, and mortality. The association has three clearly established dimensions: (a) depressed patients have a higher rate of sudden cardiovascular death; (b) depressed patients develop ischemic heart disease at higher rates than do nondepressed controls; and (c) postmyocardial infarction patients with depression have a higher cardiac mortality than medically-comparable nondepressed controls. At this point, it is unknown whether the treatment of depression will prevent the development of ischemic heart disease or reduce the associated increase in cardiac mortality. Before this question can be studied, however, it needs to be established that a safe and effective antidepressant therapy exists for patients with cardiac disease. Dr. Roose clearly explains the many dimensions of the complex relationship between depression and cardiovascular mortality, the benefits and risks of tricyclic antidepressant therapy in patients with cardiovascular disease, and the question of whether SSRIs constitute a safe and effective antidepressant therapy for patients with depression and cardiac disease.
